PLAYER RELEASE FORM

Each youth & junior team must submit a one time PLAYER RELEASE FORM for every rostered player that participates on their team for the season. This form must be completed and sent to the IHAAZ prior to a team participating in an IHAAZ qualifier.

TEAM ROSTER AND ENTRY FORM

A TEAM ROSTER AND ENTRY FORM must be submitted to the IHAAZ by a youth or junior team prior to participating in an IHAAZ qualifier. This form must include each player that is officially on the team’s roster below (Last Name, First Name, age at Dec 31, 2012 & Jersey Number). Please note… any players being added to the team’s roster as well as any players that are being removed from a team’s roster can be done after the initial roster is submitted.

Players may not be added
to rosters at Qualifiers

Youth teams will be allowed to add one skater and/or one goalie to their team
rosters, if teams choose to do so, when they arrive at Qualifiers. All youth roster adds require a $15.00 fee per roster
addition.

Players must be officially deleted from rosters

Teams must officially delete players from their team roster by indicating so on the ROSTER CHANGE section of this form before mailing in or handing over to the IHAAZ or may indicate player deletions on the TEAM SIGN-IN form that players must sign at the tournament venue.

Teams should be registered

Teams in the 8u through the Junior age divisions can be registered with AAU. Check IHAAZ website for more info on IHAAZ and AAU fees and how to register.

ADDING NEW PLAYERS TO A ROSTERR

Teams wanting to add new players to their roster once a team has already played in a qualifier, will be required to submit a $15.00 fee to the IHAAZ for every new player added to their team’s official roster. Fees are not required for teams who delete players from their roster.
